Improving health status driven by patient-centered, coordinated, healthcare partnerships serving vulnerable, resource-constrained people in southeast Tennessee and the surrounding communities.

An expanded healthcare / wrap-around service model that will utilize partnerships and technology to create a virtual one-stop shop employing best practices and community-centered care for our most vulnerable regional residents. A formalized partnership alliance of established medical and social service providers will coordinate care, including individualized case management for preventive, intervention, and rehabilitation opportunities. This focuses on reducing health disparities for chronic disease through a whole-person approach.
